NYT: Prison dementia makes caregivers of killers

SAN LUIS OBISPO, Calif. ? Secel Montgomery Sr. stabbed a woman in the stomach, chest and throat so fiercely that he lost count of the wounds he inflicted. In the nearly 25 years he has been serving a life sentence, he has gotten into fights, threatened a prison official and been caught with marijuana.

Despite that, he has recently been entrusted with an extraordinary responsibility. He and other convicted killers at the California Men?s Colony help care for prisoners with Alzheimer?s disease and other types of dementia, assisting ailing inmates with the most intimate tasks: showering, shaving, applying deodorant, even changing adult diapers.

Their growing roster of patients includes Joaquin Cruz, a convicted killer who is now so addled that he thinks he sees his brother in the water of a toilet, and Walter Gregory, whose short-term memory is ebbing even as he vividly recalls his crime: stabbing and mutilating his girlfriend with a switchblade.

?I cut her eyes out, too,? Mr. Gregory declared recently.

Dementia in prison is an underreported but fast-growing phenomenon, one that many prisons are desperately unprepared to handle. It is an unforeseen consequence of get-tough-on-crime policies ? long sentences that have created a large population of aging prisoners. About 10 percent of the 1.6 million inmates in America?s prisons are serving life sentences; another 11 percent are serving over 20 years.

While no one has counted cognitively impaired inmates, experts say that prisoners appear more prone to dementia than the general population because they often have more risk factors: limited education, hypertension, diabetes, smoking, depression, substance abuse, even head injuries from fights and other violence.

Many states consider over-50 prisoners elderly, saying they age up to 15 years faster.

With many prisons already overcrowded and understaffed, inmates with dementia present an especially difficult challenge. They are expensive ? medical costs for older inmates range from three to nine times as much as those for younger inmates. They must be protected from predatory prisoners. And because dementia makes them paranoid or confused, feelings exacerbated by the confines of prison, some attack staff members or other inmates, or unwittingly provoke fights by wandering into someone else?s cell.

?The dementia population is going to grow tremendously,? says Ronald H. Aday, a sociologist and the author of ?Aging Prisoners: Crisis in American Corrections.? ?How are we going to take care of them??

Some prison systems are confronting that now. Many would like to transfer demented inmates to nursing homes, but their often-violent crimes make states reluctant to parole them and nursing homes reluctant to take them.

New York has taken the top-dollar route, establishing a separate unit for cognitively impaired inmates and using professional caregivers, at a cost of about $93,000 per bed annually, compared with $41,000 in the general prison population. Pennsylvania and other states are giving mental health workers special dementia training.

But some struggling prison systems, including those in Louisiana and California, are taking a less expensive but potentially riskier approach. They are training prisoners to handle many of the demented inmates? daily needs.

Gold Coats, trained by the Alzheimer?s Association and given thick manuals on dementia, were the first to notice when Mr. Cruz began putting his boots on the wrong feet and ?started pulling down his pants and going to the bathroom wherever he was,? said Phillip Burdick, a Gold Coat who is serving a life sentence for beating a man to death with a hammer.

Gold Coats report these changes, often at weekly support group meetings with Dr. Steed. They identify ?different tricks and strategies to get guys to do what they need to do,? she said.

Before the program was started in 2009, demented inmates frequently caused fights, hitting those they considered threatening or disturbing other prisoners by encroaching on their turf. ?The whole atmosphere was hostile,? said Bettina Hodel, a psychologist who started the program and once narrowly avoided being struck herself. Now, Gold Coats absorb much of that behavior.

?I been swung at, got a big fat lip, and my glasses have been broken,? said Ramon Ca?as, a Gold Coat who killed a hitchhiker who stole his car. He said Gold Coats ? there are currently six of them for about 40 inmates ? often wear surgical gloves because they are exposed to ?a lot of body fluids.?

But they also protect demented inmates from prisoners who try assaulting, abusing or robbing them. ?Not responding to questions appropriately, being belligerent ? it?s just considered bad behavior,? said Sharen Barboza, director of clinical operations for MHM Services, a prison mental health provider that trains prison officials.

New York?s Unit for the Cognitively Impaired, begun five years ago, has so far cared for 84 inmates, but ?there?s a number of people in the system that we really haven?t tapped,? said Paul Kleinman, the program?s psychologist. ?They?re not being identified properly.?

Alzheimer?s currently affects 5.4 million Americans, a number expected to double by 2040. The chapter?s area director, Sara Bartlett, worried that she and Arlene Stepputat, then the program director, would not be safe as ?women in a man?s prison.? She doubted whether violent felons could provide sensitive care.

Both women were surprised that inmates seemed more receptive, with less-complicated emotional ties to the patients than many of the people they trained to care for relatives at home. ?They were much easier to work with,? Ms. Stepputat said.

Heriberto G. Sanchez, chief psychologist of the California Men?s Colony, said prisoners ?were appreciative that someone from the outside world thought they could do this.? One wrote in an evaluation, ?Thank you for allowing me to feel human.?

The prison requires that Gold Coats have ?a clean behavior record for about 5 to 10 years,? Dr. Steed said. So far, only one Gold Coat has been removed, because ?he had problems? with dementia patients? messy eating and other behaviors, Dr. Hodel said.

For inmates, the job has attractions. It pays better than other prison work and polishes a prisoner?s record.

Two Gold Coats have received parole.

One of them, Shawn Henderson, who got 25 years to life for a 1985 double murder and was twice denied parole, was released last February. Doing a job where ?you get spit on, feces thrown on you, urine on you, you get cursed out? helped teach him to cope outside prison, said Mr. Henderson, 46. ?Now when I come into an encounter like that on the street, I can be a lot more compassionate,? he said. ?And I don?t look at telling authorities as snitching anymore.?

Gold Coats conduct exercise classes and run meetings designed to stimulate memory and lessen disorientation. They escort inmates to doctors, acting as their intermediaries.

And they often need to be deft. One 73-year-old inmate stands by a gate most mornings, waiting for his long-dead mother to pick him up. Sometimes he refuses to shower, afraid of missing her. Mr. Evers coaxes him inside, telling him that his mother ?wants you to shower before she gets here.?

More subtlety is required for Mr. Gregory, 71, who is serving a life sentence for brutalizing his girlfriend with a switchblade ? throwing her body parts in the trash and getting caught, he said, when ?I went right back to the room that I killed her in and had sex with another? woman. He does not believe he has dementia, but his gradually accumulating symptoms include breaking a mop over an inmate?s head and writing to outside agencies under the delusion that he will be granted parole.

To assist Mr. Gregory, Samuel Baxter, a Gold Coat who fatally shot a co-worker, firing six times, gently reminds him about bed making and schedules. ?You have to allow Mr. Gregory to come to you,? Mr. Baxter said.

Gold Coats say they are moved by the work. ?I?m a person who was broken,? said Mr. Burdick, who during 35 years in prison lost a wife to AIDS and a 16-year-old daughter to suicide. Dementia patients often ?don?t even say thank you,? he said, but ?they just pat me like that and I know what that means.?

Mr. Ca?as said: ?I didn?t have any feelings about other people. I mean, in that way, I was a predator.? Now, he said, ?I?m a protector.?

Still, the Gold Coats have not figured out how to help Leon Baham.

When Mr. Baham, 71, received a dementia diagnosis a year ago, a psychiatrist, Dr. Russell Marks, noted that he pined for his wife ?almost as though at moments he didn?t realize? that his crime had been ?murdering the woman he was tearful about.?

In a recent interview, Mr. Baham recalled the murder hazily: ?Blood everywhere. She said, ?Sweetheart.? ?

He has repeatedly been placed in the crisis center, once after ?he urinated on the floor, he was banging his head on the cage, he needed a spit mask to prevent his spitting on others,? and he was ?threatening to kill persons that he believed stole his watch,? Dr. Marks said.

After trying to enter the wrong cell, he told Dr. Steed, ?I?m going to kill myself,? adding, ?I don?t want to live this way.?

He was sent to a psychiatric hospital, returning less depressed. But he often sits confused in the yard. ?I forget why I was going out there,? Mr. Baham said. ?I?m slipping a little bit.?

Still, he resists the Gold Coats? help and believes that he would have to pay them. Oblique assistance, like Mr. Burdick bringing him a jacket, is all he accepts so far.

?I don?t need them, you know,? Mr. Baham said.

?I Was a Monster?

Only in 2000, after Mr. Montgomery, 47, was found with marijuana, accused of threatening a prison official and locked in the ?hole,? did he decide to change. ?I was a monster,? he said.

Families of demented inmates seem unperturbed that prisoners like Mr. Montgomery now have so much responsibility. Laura Eklund, Mr. Cruz?s niece, said prison officials have asked if his relatives wanted him paroled, but the family has declined. ?To be honest, the care he?s receiving in prison, we could not match,? she said.

When Mr. Cruz spies his own reflection, he often believes it is his brother Sergio. To keep him from getting agitated, his cell mirror has been covered with tape. But now when he looks into a toilet, he calls: ?Hey, my brother, he?s down there. I can?t get him out.?

Mr. Montgomery said he tries to reassure Mr. Cruz, but if Mr. Cruz is locked in his cell, Mr. Montgomery ? still a prisoner, after all ? cannot enter even if he is allowed out of his own cell. He will call to Mr. Cruz through a tiny window in the thick metal door. ?All I can do is say, ?Cruz, come here, come here, come here,? but he?ll stand there,? staring helplessly into the toilet and agonizing. ? ?See, see, look, see.? ?

Hullo, ladies and gents! You may have wandered in here going "what the heck is she on about?" Or you might have been sent by someone, in which case, I didn't do it. Whatever the reason, you've stumbled into the OOC of a freeform Hell RPG, so of course, the rules are going to be big here. Scroll down to find those.

The Concept: You are a damned soul, or maybe you're a demon, fighting to keep yourself alive in the brutal environment provided for you by your loving parents and other higher ups. Your options are simple--or at least they should be. Climb the ranks and survive, or delude yourself and try to climb out of Hell. Either way, you'd best keep your wits about you, because other demons will be on your back soon.

General Rules and Common Courtesies

  • I don't think I need to teach anyone the rules of RP etiquette, but just in case: No god modding, power playing, mary-sue characters, or anything of the like. I'd hate to have to kick you out for something so juvenile.
  • This roleplay has a 300-word post minimum, although more is appreciated when possible.
  • If you can help it, please avoid posts that exceed 1500 hundred words. In a private RP setting, these would be favorable, but sometimes they can limit the interactions of a group.
  • Please don't ignore characters unless you specify that that is what your character is doing. While I understand this can be the intent IC, to completely ignore the fact that a player has posted is disrespectful.
  • Please make your posts in appropriate locations, and do not hesitate to repost if you have done so incorrectly. Leaving a note in your original post, such as [moved] or [moved to blah blah blah], will suffice.
  • Participate in OOC discussion and collaborate with other players. Since this is a character driven roleplay, rather than a plot driven one, it will be helpful if you discuss your ideas and information with the rest of the group, rather than trying to enact it with no warning. It is also advised that you participate in some of the world-building threads, in order to flesh out the world in which our demons are wreaking havoc.
  • A decent grasp on the English language is required for this roleplay. If you're a decent writer and can build believable characters, you will likely not be turned away.
  • When in doubt, shoot a PM to either myself or anyone who takes on a role as a co-GM. I'll probably have two or three by the time this roleplay is riding at full force. If you see that someone has a question you know the answer to, feel free to shoot them a PM and clarify things. (I guess basically I'm trying to say that everyone should work together in peace and love and harmony.)
  • Feel free to join regardless of how long the RP has been running. Someone will be along shortly to give you an idea of the things that have gone down recently, and of events that you need to be aware of before you post your character's introduction.
  • Have fun, respect each other, and keep us informed if you need to drop the RP or take a break for a while.

(AP) ? As Carnival builds toward its out-of-control crescendo of Fat Tuesday, Barry Kern and his team of float-builders and artists are already preparing for next year's parades.

One of the biggest free parties in the world fuels a multimillion-dollar industry for the city of New Orleans and the lifeblood of businesses like Kern's studio, which has been operating for more than 50 years and makes or repurposes some 400 floats a year, or roughly a float a day, Kern said.

The Mardi Gras season, which includes weeks of parades, fancy balls and parties leading up to the big day, draws hundreds of thousands of visitors to New Orleans each year, said Kelly Schulz, spokeswoman for the New Orleans Convention and Visitors Bureau. Schulz said a recent study conducted by Tulane University estimated the direct economic impact of Mardi Gras at roughly $144 million.

Some studies estimate the economic impact at more than $500 million, said Arthur Hardy, a Mardi Gras historian.

"There's no way to know for sure because we don't sell tickets," Hardy said. "Mardi Gras started small, in private homes and private balls, and it's evolved into this festival that some estimate produces more than a half-billion dollars a year."

Attendance is also hard to gauge, but every Mardi Gras hotels are full, or close to it, Schultz said.

"The city will be virtually sold out," Schulz said. "Mardi Gras and music, especially on the international scene, are our big sells."

In the weeks leading up to Mardi Gras, more than 100 parades roll into New Orleans and its suburbs. The big parading clubs, like Rex, Zulu, Bacchus, Endymion, Orpheus and Muses, hire Kern's studio to build the floats. Smaller clubs make their own by decorating trailers with everything from paint to crepe paper.

Hardy said more than 100,000 people ride in parades each year, and each rider can spend as much as $2,000 to $3,000 in fees, costumes and throws. Thousands more are spent on king cakes and the grand balls and parties, he said.

"It's a money-maker for the city, but that's not why we do it," Hardy said. "We do it because we like to celebrate. It's a free party we give ourselves and our guests."

There's big money in it. Major parade krewes often spend hundreds of thousands of dollars to have Kern's studio make their floats. Depending on whether the floats are being built from the ground up or repurposed, the price can range anywhere from $10,000 to $100,000.

Kern declines to say just how much revenue his company takes in annually. But over the years the floats have become larger and more ornate, and more expensive.

They can be as high as 18 feet and up to 50 feet long, carry dozens of riders and be wired with electricity for decorative lights and moving parts. Teams of painters, artists and sculptors make props and decorations that will be attached to the floats. Music-themed floats can include props of Louis Armstrong and local favorite Professor Longhair. Some are modeled after characters in Greek mythology, such as the Muses of dance, poetry, music and other arts.

It takes an entire year to prepare enough floats to roll through the streets of New Orleans and its suburbs, Kern said.

"It's a constant process," Kern said. "It's like an assembly line."

With the revelry of Fat Tuesday at hand, Kern's preparation for Mardi Gras 2013 has already begun.

"We already have all the designs for all our major clients for 2013, and we've already got props and things picked out," Kern said. "Literally, the day after Mardi Gras, we're back to work and the process gets started almost immediately."

Besides Mardi Gras, Kern's studio has clients in Japan, Korea and theme parks across the country.

"There are a lot of municipalities all over the world that want to copy what we do here in New Orleans because it drives tourism," Kern said.

Besides float-building, Kern's studio is a tourist attraction. Tour guides take visitors through Mardi Gras World's displays and to see sculptors and artists at work.

"I'm awestruck by some of the props," said Debra Sanders, of North Sioux City, S.D., just after her tour of Mardi Gras World recently. "It was very nice, very entertaining. I enjoyed it."

Paul Thompson, of Cheshire, England, said he was surprised by the quality of the work.

"It was very intricate and very colorful, much more professional than what you would surmise from a once-a-year Carnival," Thompson said. "It's quite amazing."

Understanding China's formidable cigarette industry - FSI Stanford

Newly printed ?no smoking? signs went up across China when the government rolled out a nationwide public indoor smoking ban in May 2011. A sticky gray layer of smoke residue now coats many signs, representing the challenges China?s growing tobacco control movement faces against a multibillion-dollar government-run industry and deeply embedded social practices.

How has the cigarette become so integrated into the fabric of everyday life across the People?s Republic of China (PRC)?

To get to the heart of this question, historians, health policy specialists, sociologists, anthropologists, business scholars, and other experts will meet Mar. 26 and 27 in Beijing for a conference organized by Stanford?s Asia Health Policy Program. They will examine connections intricately woven over the past 60 years between marketing and cigarette gifting, production and consumer demand, government policy and economic profit, and many other dimensions of China?s cigarette culture.

Anthropologist Matthew Kohrman, a specialist on tobacco in China, will lead the conference. In a recent interview, he spoke about the history of China?s cigarette industry, cigarettes and society, and the tobacco control movement.

The early years

Tobacco first entered China through missionary contact in the 1600s, says Kohrman, but it was not until the early 20th century when cigarettes began gaining popularity. The first cigarette advertising was a ?confused tapestry? of messages as marketers figured out what spoke to the public. ?There were just as many images of neo-Confucian filial piety as there were of cosmopolitan ?modern women,?? says Kohrman.

Through improved marketing and aggressive factory building, British American Tobacco and Nanyang Brothers, China?s two largest pre-war firms, helped increase the demand for cigarettes. The Sino-Japanese War (1937?1945) disrupted the cigarette supply, but their popularity had taken hold. Some cigarette firms shifted during the war to the relative safety of southwest China, where tobacco production has remained concentrated ever since.

Post-1949

After the founding of the PRC in 1949, the tobacco industry was nationalized and strong relationships between the central government and cigarette manufacturers in the provinces were formed. Cigarettes also began to be viewed as a part of everyday life. ?Ration coupons for cigarettes were issued alongside grain, sugar, and bicycle coupons,? says Kohman. ?The Maoist regime legitimized cigarettes as the right of every citizen."

During the Deng Xiaoping era (1978?1997), China?s cigarette industry really took off as manufacturers competed with one another for foreign currency to purchase cutting-edge European equipment and newer varieties of tobacco seed stock. Increased production and the return of full-scale advertising fueled greater consumer demand, and manufacturers began producing more and more varieties of cigarette. Vendors displayed glass cases filled with a colorful patchwork of cigarette packs bearing names like Panda, Double Happiness, and Red Pagoda.

The tobacco industry remained under government control as other industries privatized in the 1980s and 1990s. Party-state management of the cigarette became even more centralized in the early 1980s with the creation of the China Tobacco Monopoly Administration and its parallel external counterpart, the China Tobacco Corporation.

Since 1949, provincial protectionism has marked the cigarette market. It is now possible to purchase Beijing cigarettes in Kunming, Chengdu brands in Shanghai, and so on, but to distribute cigarettes in another province, a manufacturer must cut a deal with provincial government officials. Provincial administrations are loath to cut such deals because central government policy dictates that sales tax on cigarettes, which do not go to the central government, always return back to the province of original production. China?s 2001 entry into the World Trade Organization opened the market ever so slightly to international brands like Marlboro and Kent, but domestic brands continue to dominate because of fierce protectionism.

A new era

In 2003, the World Health Organization established the first global health treaty, the Framework Convention on Tobacco Control (FCTC). Although the United States still has not yet ratified the FCTC, China signed the treaty in 2003 and ratified it in 2005. Kohrman says China?s tobacco industry giants fear competition from international cigarette brands more than they worry about tobacco control measures related to the FCTC.

Nonetheless, the FCTC ushered in a new era of public health research about tobacco and has helped increase public awareness about the dangers of smoking. New restrictions have been imposed on print and television advertising for cigarettes, and international organizations, such as the Bloomberg Family Foundation, have begun funding anti-tobacco work in China.

A big challenge to tobacco-control campaigns, says Kohrman, is the sheer amount of money that tobacco companies have available for marketing. ?In 2010, China?s tobacco industry posted profits in excess of U.S. $90 billion?that?s huge. Tobacco control research and advocacy now annually receive a few million dollars, and much of that is coming through outside funders, which have very specific projects in mind.?

China?s tobacco advertisers have adapted to the new restrictions that prevent them from openly promoting cigarettes in the media. They have instead moved to point-of-sale and soft-marketing tactics, including misinformation campaigns about the ?dangers? of quitting smoking. ?The actual expenditure on marketing probably hasn?t dropped very much,? says Kohrman.

Cigarettes and society

Strong marketing and the legitimization of cigarettes as a part of everyday life have led to the deep integration of cigarettes into Chinese society. While only 3 to 4 percent of women in China smoke, cigarettes are an important part of male identity and social mobility. The wide range of cigarette brands has led to the growth of high-end varieties favored by businessmen and politicians, with some brands costing as much as $50 a pack. The custom of cigarette gifting has existed in China for decades, and it is difficult for a young man to turn down a package of cigarettes from a senior colleague or supervisor.

There is also the fact that nicotine is highly addictive, and quitting is difficult in an environment where smoking cigarettes is socially sanctioned. Kohrman says, ?When you take an incredibly addictive substance like nicotine and throw it into the mix of all of these norms and customs, it creates a pretty toxic brew.?

The future?

Tobacco control presents a formidable challenge in China, one that requires understanding the historical context and complex dimensions of the cigarette industry. ?Cigarettes have been insinuated into so many aspects of daily life across China, and the market for this product has now become so closely enmeshed with matters of government finance and operations,? says Kohrman.

What happens in China could have implications for the entire world. ?There?s a tobacco-induced human annihilation unfolding right now in almost every country and questions about how society and Big Tobacco are enmeshed, and how cigarette culture and government finance have become mutually supportive are pivotal,? says Kohrman. ?Every country except Bhutan has legalized cigarette sales and is subject to the same issues as China?only in China they?re on a much larger scale. But if it chooses to do so, China is in a position to lead and change the landscape in a very profound way.????
